For All Patients and Caregivers
- Select New Patient Registration if you wish to begin your enrollment into the MMP.
- Click New Patient/Caregiver Payment if you have received a payment confirmation email from the MMP.
- Click Replacement Card payment if you have received a payment confirmation email from the MMP to replace your lost, stolen, damaged card; or if you have changed your address.
For Patient and Caregiver ID card renewal
- If you have received an ID card renewal email for your expired MMP identification card, select Renew your identification card to begin the process.
- If you have received a notification email to pay for your renewed identification card, select Pay for your renewed card.
